Optimal Home Care is looking for a Physical Therapist to provide services to our patients in the Fort Collins, Greeley, Loveland, and surrounding areas. We offer astounding benefits including:Mileage reimbursementCompetitive benefits package401K planCompany phonePTO given up front (Full-time employees)Opportunity to provide individualized care and connect with patients.Continuing educationReferral bonusesEAP ProgramTablet Purchase ProgramProfessional, supportive cultureCore values: Commitment Advocacy Respect ExcellenceAbout the Position:Optimal is looking for a licensed Physical Therapist who holds a passion for improving patients' quality of life in the comfort of their placement.As a Physical Therapist you will:Comply with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.Assess each patient and developing an appropriate treatment plan with measurable goals and time frames.Demonstrate treatment skills that are rooted in evidence-based practice guidelines while balancing both patient and provider expectations and needsPay: $80,000-100,000 Optimal pays per visitQUALIFICATIONS:Graduate after successful completion of a physical therapist education program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE) or a successor organization of CAPTE, or a school approved by the American Medical Association (AMA).Preferably at least one (1) year of experience as a Physical Therapist.Must be currently licensed and in good standing in the state of Colorado.Must be able to communicate in the language of the client and document client related activity as appropriate.Possesses adequate and effective written and verbal communication skills.Possess adequate and effective interpersonal skills.IND123
