Pay: $48-$52 an hr - Bi-weekly

Job Type:

Onsite in Aurora CO 80045

Shift Options:

The work will be performed Monday-Friday, excluding federal holidays; 8-hour shift between the hours of 7am-530pm Mountain Time



RN Case Manager Overview:

Care Coordination: Assess, plan, implement, and evaluate comprehensive care plans for Veterans, ensuring seamless coordination between VA facilities and community providers. ---USAJOBS


Consult Management: Process community care consults, following them from initiation to completion, and address follow-up requests for continued or additional care needs. ---USAJOBS


Interdisciplinary Collaboration: Work closely with internal and external healthcare professionals to facilitate high-quality, cost-effective patient care. ---USAJOBS


Patient Advocacy: Serve as a liaison and advocate for Veterans and their families, ensuring their healthcare needs are met with dignity and compassion. ---USAJOBS


Utilization Management: Monitor and evaluate the effectiveness of care plans, ensuring appropriate resource utilization and adherence to clinical guidelines. ---


Documentation: Maintain accurate and timely documentation in accordance with VA policies and procedures.



RN Case Manager Qualifications:

A minimum of two (2) years direct patient care experience within the last three (3) years
Associate's or Bachelor's degree in Nursing from an accredited educational institution.
All Registered Nurse applicants must possess and maintain an active, current, full and unrestricted Registered Nurse license to practice as a RN in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
Active BLS (AHA) certification.
Proficiency in reading, writing, speaking, and understanding English.
Basic computer skills, including typing.
